About Us The Boone County Community Clinic is a State-Funded, 501c(3) non-profit Community Health Center located in lebanon, Indiana. We use the most innovative, up-to-date Health Information Technology to ensure we provide the best care to all of our patients, all of the time.
Mission The Mission of the Boone County Community Clinic is to provide greater access to basic medical services to Boone County residents who have limited Financial resources or healthcare reimbursement. We offer services to Uninsured and Underinsured residents of Boone County.
